Obtaining Copies of Birth Certificates

Where do I obtain a birth certificate copy for someone born in New York State outside of New York City?

• In the city of town in which you were born.

Who is eligible to obtain a birth certificate copy?

• The person named on the birth certificate.

• A parent of the person named on the birth certificate (requesting parent's name must be on birth certificate).

• Only by order of a New York State Court may a spouse, child or other persons obtain a copy of a birth certificate.

What is the fee for a birth certificate copy?

• The fee is $10.00 per birth certificate copy

• Payment of mail order copies may be made by money order payable to the Town of Poughkeepsie.- Do not send cash

How long will it take?

• Mail requests ordered with regular handling (or requests submitted via first class, registered, certified or two-day mail) will be processed within 2 days from the day when they are received in the Clerk's office.

How do I place an order for a birth certificate copy?

• Order in person with a valid identification

Identification Requirements:

Application must be submitted with copies of either A or B

1. One (1) of the following forms of valid photo-ID

• Driver license

• Non-driver license

• Passport

• Employment ID

2. Three (3) of the following showing the applicant's name and address

• Utility or telephone bills

• Letter from a government agency dated within the last six (6) months

• Order by mail

Send money order made out to the Town of Poughkeepsie for $10.00 and a letter with the birth name, date of birth, father's name, mother's name with maiden name, place of birth, and have signature on letter notarized. Also include a return address (no P.O. Box number) to:

Remember, the signature on the letter must be notarized.
